Athletic Clearance Forms

Students must complete the Athletic Clearance Packet prior to participation.

Clearance forms and procedures may be accessed ONLINE through SportsNet (preferred method),  PDFs can be printedor they can be picked up in person in the Health Office. The Health Office is located inside the Main Office on the Student Services/North side. Paper packets/forms must be returned to the Health Office. Please contact our Health Aide, Lesley Franco, with any questions related to athletic clearance.

Academic Eligibility

Student-athletes for 2020-2021 must have at least a 2.0 GPA (unweighted), be passing at least 20 credits (4 classes), and have no more than 1 failing grade to be academically eligible to participate in contests. Eligibility criteria is evaluated for the most recently completed grading period (six weeks) and is not based on a student’s cumulative academic performance. Students entering from middle school are automatically eligible for the first 6 weeks of the fall season. 

 

Eligibility dates indicate the days upon which changes in eligibility status take effect. The eligibility dates for 2021-2022 are TBD.

 

Transfer Students

All transfer students from other schools need to complete paperwork before you can participate in athletic contests.  Please contact the Athletic Director, Steve Sell, for assistance. 

Summer Workouts

All students who wish to participate in summer workouts with any of our athletic teams must be medically cleared to participate. This includes returning students as well as incoming freshmen.
